This Telehealth Informed Consent describes the nature of telehealth care at Helix Peptide Health. You will review and acknowledge a final version of this consent on the clinical platform before your consultation with Dr. Jamie Luria, MD.
What telehealth is
Telehealth means the delivery of medical care using secure video, audio, and messaging rather than an in-person visit. Your consultation, follow-ups, and communication with Dr. Luria take place through a secure, HIPAA-compliant platform.
Benefits and limitations
Telehealth can improve access and convenience. It also has limitations: a physician cannot perform a hands-on physical examination, and some clinical situations are not appropriate for telehealth care. Dr. Luria will tell you if your situation requires in-person evaluation or additional testing.
Your care and privacy
Care delivered through the clinical platform is protected under applicable privacy laws, including HIPAA. Information you share as a patient is used to provide and coordinate your care.
Your consent
By proceeding on the clinical platform, you consent to receive care via telehealth, understand its benefits and limitations, and understand that you may withdraw consent at any time.
Emergencies
Telehealth is not for emergencies. If you think you may have a medical emergency, call 911 or go to the nearest emergency room.
