It is required by the Health Insurance Portability and Accountability Act (HIPAA) and is distinct from the website Privacy Policy. A final version will be provided and acknowledged during your first clinical encounter.

How to file a complaint

If you believe your privacy rights have been violated, you may file a complaint with the practice or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
